PHARMAC has a critical job to do on behalf of the New Zealand public in achieving the greatest health gains it can from the pharmaceutical budget. The decisions it needs to make are often difficult and are often driven by the high or increasing costs of certain groups of drugs. The three major initiatives involving cardiovascular drugs have provided valuable lessons on the process of developing and implementing important pharmaceutical subsidy decisions. It is time to turn those lessons into action. We consider that PHARMAC's decision-making processes need to be explicit and transparent, and have suggested a number of fundamental principles which, we believe, would improve them. Ometimes you find yourself in a mess through no fault of your own. On other occasions the problem is self-inflicted, because of character traits that have troubled you in the past and no doubt will do so again in the future. One such character trait that I possess is a tendency to be flippant when relaxed. Well, I was very relaxed on a lovely summer evening a year or two ago in north America. The day's work was over, and I was in happy hour mode; I sensed upregulation of my flippancy gene. The purpose of my visit was to attend the annual meeting of the American Endocrine Society AES ; . As far as I could tell, my presentation had been well-received, and I was now enjoying myself at a dinner held for a small constituent society of the AES that always arranges a social function to coincide with the annual meeting. I was searching for my second pre-dinner cocktail when I spotted Mike, an American colleague. I had known Mike the name has been changed to protect the guilty ; for years: a lively, intelligent man of strongly held beliefs, and equipped with a great sense of humour. We had sparred many times before in a jocular good-hearted fashion. Furthermore, I was certain that he had just refereed one of our articles for Clinical Endocrinology. The style, the manner of expression, and the content of the referee's argument all pointed to Mike. The article was a tricky one and I have to admit that I was amazed that my research fellow had ever conjured a manuscript out of such raw data. Nonetheless, he had done a very professional job, and no other data on this topic exist in the literature. So we were in the position of the one-eyed man in the land of the blind, and I anticipated that the journal would accept the manuscript. Referee one, whom I guessed was Mike, started down the first page of his report by not liking the article and by the end of the page he hated it; boy, did he hate it! Vitriol was pouring from his pen as he became more and more agitated by our efforts. The journal behaved impeccably and allowed us to rebut the criticisms comments, eventually accepting and publishing a revised version of the manuscript. I had to pass in Mike's direction to get to the bar so I paused to greet him. `How are you?' `Fine.' `Tell me, why did you hate our work with such intensity?' flippancy in overdrive ; `What are you talking about?' my pulse rate quickened ; `You know, our recent manuscript for Clinical Endocrinology on Schruckelgrber's disease?' `I don't know what you are talking about, I have never refereed any manuscript of yours.' beads of sweat appeared on my forehead ; `.but YOU refereed an article of mine recently that had been sent to JCEM.' legs felt leaden ; `Did I?' I scrambled my thoughts together; was he right? Oh God, I do seem to remember an article, the article presumably. I continued, brain no longer driving my side of the conversation. `What happened to that article?' `Oh, it was rejected on your recommendation.' my need for that drink grew desperate ; `How do you know it was me that refereed your article?' legs no longer capable of movement in any direction ; `The journal sent me the referee's comments with your name on the fax.' Well for the remainder of the AES meeting, as luck would have it, I ran into Mike every day. Without fail he reminded me that I had rejected his article. Even if on escalators moving in opposite directions, and too far apart for dialogue, he would simply look across at me with a baleful eye and then give me a thumbs-down sign. I was particularly upset that my ability to referee-spot was not as good as I thought. In the past I used to.

The high prevalence of constipation in patients with cancer has been described in the preceding article.1 This condition can produce some of the most distressing symptoms of cancer and cancer treatment, thus appreciably affecting quality of life, daily living, and self-esteem.2 Untreated constipation may contribute to increased abdominal pain and distention, urinary retention, nausea, vomiting, and anorexia, as well as the development of hemorrhoids, anal fissures, perianal abscesses, and intestinal obstruction.2-4 Anal fissures and perianal abscesses can lead to systemic infections in neutropenic patients.2 This review focuses on the pharmacologic management of constipation as it pertains to cancer patients.

Mediate except at L5, which has only a medial and lateral branch ; 10. The most important of these branches is the medial branch, which hooks medially around the base of the SAP at the inferior most aspect of the intervertebral foramen, supplying the z joints just above and below it's course. Given the anatomy of a spinal nerve and its branches, a selective epidural injection will partially anesthetize the dura including the dural nerve root sleeves ; up to two segments caudally and one segment rostrally, the PLL and intervertebral disc at the same level as the nerve and one segment rostrally, and the z-joints at the same level of the nerve and one segment below. If a selective epidural injection is being done for radicular pain the fact that these structures are also anesthetized is irrelevant, as they are sources of axial pain, not radicular pain. However, if an injection is being done for axial pain, it is important to realize that pain relief after an injection can occur with pathology in any of the structures innervated by the nerve. A selective epidural injection can potentially be helpful in diagnosing axial pain19. Any lesion that affects a nerve root also necessarily affects it's dural sleeve, and therefore is a potential cause of axial pain. In the case of HNP, disc material in the epidural space has been demonstrated to elicit an inflammatory response not only in the nerve root but also the dura. Relief of both axial and radicular pain with a selective epidural injection suggests that the same lesion is responsible for both. However, one cannot be certain that the axial component of the pain is arising from structures above and or below the injected level. In patients with predominantly axial pain, fully characterizing the source of pain may require synovial joint and or disc injections, depending on the clinical situation. A selective epidural injection has two components. The first is the provocative pain response resulting from contrast injection, and the second is the analgesic response resulting from injection of local anesthetic and or corticosteriod. In evaluating provoked pain it is important to compare the location and character of the provoked response to the patients typical symptoms. Furthermore, the onset of provoked pain should be related to where the leading edge of the contrast solution is when pain begins19. Normal epidural tissue is not painful when gently stimulated by contrast solution. In the absence of scar tissue, pain provocation indicates that the tissue stimulated is irritated. For example, early pain provocation of pain, when contrast is still in the foramen, suggests foraminal stenosis or a foraminal disc herniation. Late pain provocation, when the contrast approaches the disc above, is consistent with a paramedian disc herniation. Immediately after the injection the effect of the local anesthetic injected on the patients symptoms should be assessed, both at rest and in response to mechanical stimulation. Studies on selective nerve root injections have used the criterion for a positive analgesic response to be from 80%19 to 100%22 relief. The significance of lesser degrees of pain relief in response to an injection is uncertain. If corticosteriod is included in the injection the patient should be re-evaluated one week afterwards, as the degree of pain relief at that interval can provide important information. As is the case with provocative discography, there are a number of factors that can lead to both false positive and false negative results from selective epidural injections. These factors include changes in CNS nociceptive processing that occur with chronic pain, psychological factors, and placebo responses83, 104. However, in contrast to provocative discography, selective epidural injections are done primarily for radicular pain. Therefore, there is a well established gold standard, surgical exploration, which can be used to compare diagnostic tests to and ursinus.
