What do you consider to be your greatest contributions to ASAM and the field of Addiction Medicine? I board certified in Family Medicine but I have been practicing Addiction Medicine for 46 years. I joined ASAM then called the American Medical Society on Alcoholism ; in 1968. I was first certified by ASAM in 1986, recertified in 1996, and a Fellow of ASAM FASAM ; . I was the recipient of the ASAM Annual Award in 2006. I a member of the Research Society on Alcoholism and the International Society for Biomedical Research on Alcoholism, and a Clinical Associate Professor in the Department of Psychiatry and Behavioral Sciences of the University of Washington School of Medicine. I have always promoted addiction medicine in educating medical students, nurses, and addiction counselors. I was one of the faculty of the Curriculum Committee that made Addiction Medicine one of the core constituents of the curriculum at the University of Washington School of Medicine. I also was adjunct professor for the alcoholism training program at the University of Washington School of Nursing and adjunct professor of the addiction studies program at Seattle University. How do you feel your election would benefit ASAM and the field of Addiction Medicine? I have served on many committees of ASAM, including the Public Policy Committee and the Clinical Practice Committee. Based on my corporate experience, however, I convinced that no organization will survive unless it has adequate financial support and sound fiscal management. Therefore, I have been a long-time member of the Finance Committee and, in the past, the Treasurer of ASAM. I believe that my experience on the Board of Directors and my experience in financial management will help ASAM reach its goals. Sure the safety of the product: 1 ; careful selection of donors, with importance placed particularly on voluntary, unpaid donations; 2 ; screening of every donation for hepatitis B surface antigen, anti hepatitis C virus antibodies, antihuman immunodeficiency virus 1 and 2 antibodies, syphilis serology, and normal liver function; and 3 ; use of viral inactivation procedures in addition to the already high viral inactivation afforded by cold ethanol fractionation. However, it is important to recognise that another important factor influencing the extent of energy deposition is the nature of tissue through which the waves pass. A clinically relevant consequence of these boundary conditions arises when one attempts to deliver microwave energy to a tumour with an overlying fat layer. Aimed first at the underlying cause, which is easier said than done. In serious trauma or burns, the damaged vascular endothelium often cannot be removed. Step two in therapy is replacement of consumed blood constituents, primarily platelets, fibrinogen, and Factors V and VIII. This requires transfusion of platelet concentrates, fresh frozen plasma, and occasionally cryoprecipitate. The goal of replacement therapy is to maintain a platelet count of at least 100, 000, a normal aPTT, and a fibrinogen level of at least 200 mg dl. The third possible step in therapy, the use of heparin, is reserved for disorders of the coagulation cascade and is not appropriate in surgical patients. HYPOTHERMIA Hypothermia is almost unavoidable in trauma patients because most are hypothermic before they reach the operating room, and anesthesia further compromises their thermoregulatory mechanisms. General anesthesia lowers the threshold temperature at which hypothermic corrective responses begin. Anesthesia also reduces cutaneous vasoconstriction, which normally helps to conserve heat, and muscle relaxants block the shivering response. Hypothermia has many physiologic effects, some of which may be beneficial in the critically ill. Metabolism decreases by 8% per degree Celsius, and reaches one half of normal values at 28C. Oxygen demand is reduced correspondingly so that low organ perfusion may be better tolerated. Blood flow is also shifted away from the extremities and muscles, with a larger fraction perfusing the heart and brain. Decreased cardiac output accompanied by sinus bradycardia may have a beneficial effect on the myocardial oxygen supply-demand balance. These potential benefits of hypothermia are generally outweighed by its deleterious effects in critically ill patients undergoing surgery. In addition to sinus bradycardia, the ECG may show atrial fibrillation, prolonged PR and QT intervals, and widened QRS. Below 28C, more serious dysrhythmias ensue, including nodal rhythm, frequent premature ventricular contractions, atrioventricular block, and refractory ventricular fibrillation. Increased sympathetic tone can cause heart rate and blood pressure to increase under light levels of anesthesia. Myocardial contractility increases as temperature falls, reaching its maximum at 28C. Hypoxic pulmonary vasoconstriction is attenuated by hypothermia and at 30C is less than one half of normal response. Plasma volume is decreased because of an apparent transcapillary fluid shift, thus worsening preexisting hypovolemia. Blood viscosity is increased by both the direct effect of temperature and hemoconcentration as a result of decreased plasma volume. Of special concern in trauma patients, hypothermia causes platelet dysfunction and throm. Lated glucose transport activity in insulin-resistant skeletal muscle by interacting with these early steps in insulin signaling, which are essential for the activation of insulin-mediated GLUT-4 translocation and glucose transport 5 ; . We have also addressed the potential role of NO as mediator of bradykinin action on insulin-stimulated glucose transport in insulin-resistant skeletal muscle Fig. 3 ; . The acute effect of bradykinin on insulinstimulated muscle glucose transport was completely prevented by pretreatment with the NO synthase inhibitor L-NAME. This strongly suggests that, in this animal model of insulin resistance, bradykinin brings about an increase in NO production or action that is essential for the increase in insulin-stimulated glucose transport. Whether this NO is of endothelial or myocellular origin could not be determined with this experimental design. In addition, based on the findings of Carvalho et al. 4 ; , which indicated that bradykinin can upregulate the early steps in insulin signaling in skeletal muscle of insulin-resistant, aged rats, one might hypothesize that bradykinin-induced NO production can positively modify the early steps in insulin signaling in insulin-resistant skeletal muscle of the obese Zucker rat. We are currently testing this hypothesis in our laboratory. NO can be synthesized by skeletal muscle 1 ; . The majority of previous investigations have indicated that this molecule likely is important in modulating insulinindependent glucose transport 1, 2, 25, ; , although one report supports a unique role in skeletal muscle glucose transport modulation that is both insulin and contraction independent 9 ; . Our finding that L-NAME alone caused a decrease in basal, insulin-independent glucose transport activity in muscle without any effect on insulin action see RESULTS ; is consistent with the concept that NO may be important as a regulatory factor in basal, insulin-independent glucose transport. However, it is important to emphasize that in the present study the inhibitory effect of L-NAME on the insulin-dependent pathway for stimulation of glucose transport was only observed when this process was augmented as a result of bradykinin treatment Fig. 3 ; . Collectively, these findings suggest that NO can indeed modulate insulin-independent glucose transport activity directly, but they also indicate that NO has an additional role as a mediator of the beneficial effect of bradykinin on the insulin-dependent glucose transport activity in skeletal muscle of the insulin-resistant obese Zucker rat. In the present experimental design, the whole animal was acutely treated with either ACE inhibitor, bradykinin, ANG II receptor antagonist, or NO synthase inhibitor, and subsequently glucose transport activity in skeletal muscle was assessed in vitro to eliminate the known effects of these compounds on blood flow reviewed in Refs. 3 and 8 ; . Our results support the concept that ACE inhibitors or bradykinin can modulate the muscle glucose transport system itself, consistent with previous studies using in vitro treatment of skeletal muscle 19, 20 ; or L6 myocytes and trizivir.
