Abstract Background. Recent developments have proposed the cyclosporin CsA ; concentration at 2 h post-dose C2 ; as the best single time-point predictor of the extent of CsA exposure and as the optimal basis for monitoring immunosuppressive therapy in renal transplant patients. The present study sought to validate the cornerstones of the current concept of C2 monitoring. Methods. We assessed the predictive value, dose proportionality and intrapatient variability of C2 levels in 41 de novo renal transplant recipients treated with CsA microemulsion, steroids, mycophenolate sodium and basiliximab. Results. Patients with rejection and patients with CsA nephrotoxicity had lower C2 P NS ; and absorption P 0.05 for toxicity ; , while C0 did not show any significant difference. Receiver operating characteristic analysis did not detect discriminative C2 values as a predictor of rejection or toxicity. In a substantial number of patients 29% ; we observed poor and or slow absorption, with C0 300 ng ml and C2 levels 800 ng ml during the first month and a high rate of complications in these patients 18% rejection, 64% toxicity ; . Absorption increased over the first month post-transplant. Analysis of dose changes indicated that C2 levels are not doseproportional. Intrapatient variability of C2 was as high as that of C0. Conclusions. C2 levels do not predict rejection or toxicity. C2 monitoring alone does not detect toxicity in poor and or slow absorbers, who constitute a significant proportion of patients. Changes in absorption over time, high intrapatient variability and lack of dose proportionality constitute further limitations of the C2 monitoring concept in the early post-transplant phase.

Indeed, most anticancer and antibacterial drugs approved by the US Food and Drug Administration over the last 20 years are either based on, or derived from, natural products 7, 8 ; . Marine species have become of increasing interest to bioprospectors for Western medicinals. Over 14 000 new chemical entities have been identified from marine sources 9 ; , and at least 300 patents have been issued on marine natural products 10 ; . The earliest known example of such exploration was the 1950s discovery of spongothymidine and spongouridine from the sponge, Tethya crypta, which led to the development of antiviral, anticancer, and anti-HIV drugs 4 ; . Today, a new generation of marine-derived pharmaceuticals are set to enter the market. Examples include a chronic pain treatment, Prialt ziconitide ; --which is based on a peptide isolated from the cone shell, Conus magnus 11 ; --and the anticancer drug Yondelis ecteinascidin 743 ; , from the ascidian Ecteinascidia turbinata 12 ; . Bioprospecting for pharmaceuticals has been represented typically as an environmentally benign form of resource development that would simultaneously increase our understanding of biodiversity 13 ; and help fund its conservation, especially in developing countries 1416 ; . It has been argued that developing nations with high marine biodiversity could benefit financially from granting access for bioprospecting and indirectly by building industry capacity through joint ventures. The dynamics of a thin film of viscous fluid on the inside of a cylinder with horizontal axis, rotating about this axis, are examined in this paper. The stability of this film has been previously explored using the leading order lubrication approximation, under which it was found to be neutrally stable. In this paper, we examine how the stability of the film, affected by higher order corrections, such as inertia described by the material derivatives in the Navier-Stokes equations ; , surface tension, and the hydrostatic pressure gradient. Assuming that these effects are weak, an asymptotic equation is derived which takes into account these effects as perturbations. The equation is used to examine the stability of the steady-state distribution of the film around the cylinder rimming flow ; with respect to linear disturbances with harmonic dependence on the axial variable and on time normal modes ; . In two-dimensional motion Benilov et al. ; , the linearised problem admits infinitely many harmonic solutions, all of which are neutrally stable. On the other hand, there are non-harmonic solutions that "explode" develop singularities ; in a finite time. Also the leading order three-dimensional case has been investigated, Benilov ; , where it was shown that there are infinitely many normal modes harmonic in the axial variable and time ; that are neutrally stable and where their eigenfunctions form a complete set. Benilov showed that the film is nonetheless unstable with respect to non-harmonic disturbances, which develop singularities in a finite time. The collapse of the solution always takes place in a single dimension. In Benilov et al. , the collapse occurs in the -direction, while in Benilov , it is the z-direction where the collapse occurs. Hence the aim of this paper is to see whether or not the solution can collapse in both dimensions, and if so, then what are the characteristics of that type of "explosion.

Differential blocking of cyclins indeed occurs in cells undergoing apoptosis by ATO and in cells expressing non-functional p53 Gazitt, unpublished results ; . Our interpretation of the effect of ATO in vitro could explain the low toxicity observed in patients undergoing treatment with ATO, since G1 block, unlike G2 block is less toxic to the cells and can be reversible. 2. P53-dependent differential caspase activation in ATO-induced apoptosis Our findings suggesting a p53-dependent apoptosis in cells treated with ATO is further supported by the results obtained from studies of the caspase cascade activation by ATO in cells expressing w.t. vs. mutated p53. Thus, using caspase blocking peptides, caspasespecific substrate peptides and Western immunoblotting, we clearly show that in cells expressing functional p53 the initiator caspase 9 is the principal caspase activated by ATO leading to caspase 3 activation and apoptosis. In contrast, in the absence of functional p53, caspase 8 and 10 are the principal caspases activated by ATO leading to caspase 3 activation and apoptosis. Interestingly, Seol et al. reported activation of caspase 9 in PCI-1 cells 19 ; , whereas, Kitamura et al. reported activation of caspase 8 in ATO-induced apoptosis in gastric cancer cells 20 ; . If the difference between these 2 cell lines is the status of p53 than our results explain the seemingly conflicting reports by these 2 groups for caspase activation by ATO. Caspase 8 and 10 are the primary caspases involved in the well characterized extrinsic apoptosis pathways attributed to the fasL and TRAIL 24, 51-52 ; , whereas caspase 9 is the primary caspase involved in the intrinsic mitochondrial pathway 53 ; . In rare cases the 2 apoptotic pathways could be linked through activation by caspase 8 of the proapoptotic protein, Bid resulting in its translocation to the mitochondria and apoptosis via mitochondrial damage 54 ; . It possible that ATO might be involved in triggering both apoptotic pathways since results from our lab clearly suggest that depolarization of mitochondrial membrane potential and primaquine.

MACROSOMATIC ILLUSIONS IN MULTIPLE SCLEROSIS Timothy Galey, Colleen Miller, Michael Nazareth, Rohit Bakshi, Bianca Weinstock-Guttman, Michael Santa Maria, Carol Brownscheidle, and Lawrence Jacobs, Jacobs Neurological Institute, Buffalo, NY Sensory abnormalities including numbness, paresthesia, and loss of sensation, pain, and temperature are common in individuals with multiple sclerosis MS ; . We conducted patient interviews using phenomenologic methodology to assess the occurrence of macrosomatic illusions in MS. These illusions are defined as sensations of "largeness" accompanied by banding and or tightness in a specific body region. During the course of their routine follow-up evaluations in our outpatient MS clinic, 72 patients described various macrosomatic illusions which they experienced throughout the course of their MS. Fifty-two of the patients 72% ; were female and 20 28% ; were male. The majority reported macrosomatic illusions in more than one site: 45 reports involved the lower extremity; 36 reports included the abdomen, chest and or torso; and 26 reports involved the upper extremity. There were fewer occurrences involving the face nine reports ; and only four instances in which the illusion affected an entire side of the body. The illusions were frequently described as a thickening or band of tightness in the area affected--"toes are like sausages, " "feels like I'm wearing clown shoes, " "like I have a tight corset on, " "fingers feel swollen." We are currently conducting follow-up evaluations to identify possible correlations between the illusions described, sensory abnormalities by objective neurological evaluation, exacerbations, disability status, and MRI of the head and spine. The cause of macrosomatic illusions is unknown but may result from distorted perceptions due to sensory loss as has been suggested in phantom limb phenomena in persons with MS.

CONCLUSIONS: Patients with the greatest degree of functional impairment, as measured by their FEV1, presented a higher probability of having an isolation of P. aeruginosa or H. influenzae in significant concentrations in sputum during an exacerbation. The diagnostic yield of sputum in patients with an FEV1 50% was low, with a predominance of non-PPMs. Low FEV1 and active tobacco smoking are data that should be considered when establishing an empiric antibiotic treatment for exacerbated COPD. In recognition of this problem, drug utilisation evaluation DUE ; has been recommended as a method for identifying inappropriate or unnecessary drug use and for promoting rational therapy. Growing concern over the widespread misuse of antibiotics, together with the emergence of antimicrobial resistance and escalating expenditures, has resulted in antibiotics being the drugs most frequently chosen for DUE projects. Cephalosporin DUE is well documented as being successful for modifying cephalosporin use and for containing drug expenditure. Studies range from isolated projects to ongoing programmes that comprehensively evaluate cephalosporin use and the impact of corrective strategies. Sensible use of antibiotics requires a clear understanding of the infectious process, the clinical pharmacology of anti-infective agents and an appreciation of clinical and microbiological monitoring and assessment.Audit criteria that incorporate the above principles, and which are described in the studies reviewed in this article, will be useful for other investigators. Through its DUE programme, the Royal Adelaide Hospital has investigated the use of cephalosporins, including ceftriaxone, ceftazidime and cefoxitin.These reviews have resulted in improvements in cephalosporin use and significant cost savings. Alterations to cephalosporin use that were recommended following these reviews have not resulted in adverse changes to postoperative infection rates, clinical outcomes or adverse drug reactions.
