ABSTRACT To characterize the mechanisms involved in the intestinal absorption of the essential amino acid L-lysine from the diet, the transepithelial transport of L-lysine was studied in monolayers of cultured human intestinal cells Caco-2 ; grown and differentiated on microporous membrane supports. L-lysine was trans ported mainly in the apical AP ; to basolateral BL ; direction and the BL to AP transport was ~~ one order of magnitude lower at all concentrations tested. Non linear regression analysis of the transport in the AP to BL and the BL to AP direction identified, in both cases, single saturable components with similar Km but dif ferent Vmaxand a nonsaturable diffusional component. The AP to BL L-lysine transport was highly energy- and sodium-dependent and was unaffected by an unfavor able concentration gradient. Selective replacement of sodium ions in the AP or the BL compartment and determination of both AP to BL transport and the intracellular soluble lysine pool showed that uptake oc curs via a sodium-independent mechanism, not signif icantly influenced by membrane potential, whereas ef flux is a sodium-dependent process. Competition experiments showed that L-lysine uptake is highly stereospecific and is shared by cationic and large neutral amino acids. This study demonstrates the presence of a sodium-dependent mechanism of lysine efflux across the BL membrane of intestinal cells, which may be es sential for lysine transport into the blood circulation. In this eross-sectional survey ol men aged 30-46 years n 45, 380 ; in South West Norway, respiratory symptoms, smoking habits and martlal status were obtained lrom sen-administered questionnaires and measurements of FEV1 and FVG were performed using dry-wedge bellow spirometers. The overall attendanee rate was 65%. Spirometrie reeordings were obtained in 91% ol the attendants n 26, 803 ; of whom 1.6% failed to obtain reeordings within the 1983 eriterion while 9.5% lailed the 1993 eriterion. Spirometrie lailures using both eriteria were more prevalent in single men, never-smokers and subjects with respiratory symptoms than in ever~ married, ever-smokers and asymptomatic subjects. respectively. Spirometric failure using the 1993 eriterion also inereased with age, bodyweight and declining body-height. Almost all respiratory symptoms were related to spirometrie failure using the 1993 emerion after adjusting lor demographie variables and smoking, whereas only breathlessness uphill was related to the 1963 eriterion. Although the CPT code set does not have codes that completely describe physician to physician communication for circumstances in which a primary care physician and a hospitalist are both providing care for a patient, the lack of appropriately descriptive codes is not responsible for payment or communication problems, and the development of an appropriate code would not eliminate them. The primary source of difficulty is the payment policies of third party payers that mandate the use of hospitalists and restrict payment to primary care physicians. As was discussed in testimony before the Reference Committee, payers have adopted these policies to save money and the existence of appropriate CPT codes for physician to physician communication would not change the fiscal orientation of payers. Such CPT codes would simply be ignored and not paid. The routine denial of payment for the above telephone call codes are an example of this. The only resolution to communication, payment and potential patient care problems is the adoption by payers of existing AMA policy that supports the value and central role of the primary care physician when their patient is admitted to the hospital. Such policy would allow the primary care physician to submit CPT case management codes, but more importantly would permit the primary care physician to submit claims for inpatient Evaluation and Management codes 99217 - 99239 ; on days they provide care to their patient. For example, AMA Policy H225.960 states; "the assignment of responsibility to the hospitalist physician must be based on the consent of the patient's personal physician and the patient." Such consent would help to eliminate confusion for both physicians and plans and lysine.
