Endothelial dysfunction and abnormal vascular responsiveness to vasoconstrictors may play an important role in the pathogenesis of chronic heart failure. Our study demonstrates that the vascular response to norepinephrine is abnormal in a rat model of heart failure. The increased responsiveness of intact vessels is endothelium-dependent and is the result of decreased basal release of nitric.
Due to a national shortage of injectable flu vaccines, and the restrictions put on the use of available flu vaccines, the federal Centers for Disease Control are recommending that other groups use FluMist. The parents of a seven year-old girl sued the makers of Children's Motrin and several companies that distribute the painkiller, claiming their daughter lost her eyesight and suffered other severe side effects from the medication. Motrin is manufactured by New Jerseybased healthcare company McNeil Consumer & Specialty Pharmaceutical, a subsidiary of Johnson & Johnson. In the lawsuit, the parents allege that Motrin caused their daughter to develop two disorders--Stevens Johnson Syndrome and Toxic Epidermal Necrolysis. The injured child took Children's Motrin drops on September 8, 2003 after she came home from school with a fever. The child had no known drug allergies, according to the lawsuit filed in the Los Angeles County Superior Court. The next morning, she woke up with a high fever and other symptoms, including a pink coloration in her eyes and sores in her mouth. She was hospitalized and shortly thereafter, she was blind in both eyes. Her doctors concluded that the Stevens Johnson Syndrome was caused by the Children's Motrin. Since that time the child has undergone multiple eye surgeries. The lawsuit alleges the company knew of the connection between Motrin and Stevens Johnson Syndrome from their own clinical tests dating back to the late 1980s and included warnings of the risk of this disorder with the drug before it became avail.

As a result, interest is growing in a system-wide analysis using a multi or intermodal framework as the reference point for evaluating the efficiency of transport chains. This approach stresses not only the performance of individual elements and linkages between them but also the need for system-wide efficiency measures. Performance in this context needs to be assessed having regard to the technical, pricing and dynamic efficiencies of the overall supply chain. Efficiency in intermodal transport requires each mode and each terminal comprising the total supply chain to operate in an efficient manner. To achieve this, each component of the total freight transport chain must perform at that level which achieves maximum output level for the lowest cost. Overall efficiency is obtained when these individual participatory efficiencies are combined in such a manner that holistic efficiency is achieved. Performance efficiency relates to how well the resources are being utilised. Performance effectiveness relates to how well the goals of all the participants shippers customers operators ; of the total transport chain are being attained. Efficiency is a minimiser or maximiser concept. Performance effectiveness relates to desired achievements of the participants. Emphasis on customer service requirements such as price costs, timeliness, loss and damage or any other goal that each of the participants perceives to be important ; will differ between participants and over different time periods. Thus, in some instances, the supply chain may be efficient but not effective: i.e. there are no wasted resources, but major goals of key participants are not being achieved. Interdependencies of the holistic supply chain When a transport chain is viewed in a holistic manner, maximum efficiency will only be achieved if all modal and terminal operators are providing maximum service at minimal cost. If any service provider is not attaining the optimum at their particular link of a particular transport chain, then the entire intermodal freight service will fall below maximum efficiency. Further, when all participants of the container transport supply chain are operating efficiently, there are compounding benefits that result in higher supply chain efficiencies.

And activate thermogenesis. Whereas insulin is secreted in acute response to meals, leptin stimulation is delayed for several hours [35, 36]. Therefore, leptin may be involved in longer-term energy intake control. Insulin and leptin stimulate central pathways and are secreted in proportion to body adiposity, although circulating levels are also influenced by recent energy intake [30, 37, 38]. The action of leptin is by the hypothalamus, stimulating energy expenditure from basal metabolism and reducing the drive to eat [11], and potentially inhibiting the effects of the orexigenic hormone ghrelin [39, 40]. Inter-relations among insulin, leptin, and ghrelin may provide mechanisms through which short-, medium-, and long-term energy balance controls are linked. Ghrelin, which is secreted primarily by the stomach, has been implicated in the pathogenesis of obesity [22, 41, 42]. In rodents and in humans, ghrelin has been shown to increase before meal initiation, leading to hunger and food intake, and it subsequently declines after food ingestion [17, 21, 22, 41]. However, in humans, higher body weight is associated with lower ghrelin levels [13], and diet-induced weight loss is associated with elevations in plasma ghrelin, which may promote recidivism [41]. These data indicate a critical role of ghrelin in energy balance, and its relation to dietary factors deserves further research attention. Consumption of high-fat low-carbohydrate meals results in lowered 24-h circulating leptin concentrations in normalweight women compared with low-fat high-carbohydrate meals [40]. Leptin has also been shown to be reduced after consumption of meals accompanied by beverages sweetened with fructose, a sugar that induces low glycemic responses, as compared with glucose [16]. In the present and focalin.
Increase influenza vaccination among children 5 to 17 years of age a group that frequently transmits influenza disease. Similar programs will be conducted in certain school districts throughout the United States. MedImmune has worked closely with State Assemblywoman Sally Lieber to bring the FluMist for Schools initiative to the Bay Area. MedImmune will provide enough FluMist for schools in Santa Clara and Sacramento Counties to arrange for the vaccination of more than 2, 200 schoolchildren and faculty. The vaccination program is voluntary, and will be offered at certain schools in Santa Clara and Sacramento Counties for a limited time beginning on October 17. The counties will arrange for FluMist to be administered by a licensed professional to students during designated "vaccination days" at each participating school. Parents who wish to have their child vaccinated with FluMist must submit a signed consent form to the school's health care administrator. FluMist is indicated for healthy children aged 5 to 17, and healthy adults aged 18 to 49 years. Once infected with the influenza virus, children can easily transmit the virus to their parents, siblings and other members of the household. Studies have shown that when school-aged children are vaccinated against influenza, the overall incidence of influenza cases decreases in the household and in the community. A school-based influenza immunization program may be a convenient system for immunizing large groups of school-aged children during the fall and winter months. Those interested in learning more about whether FluMist is a vaccination option for themselves or their school-aged children should contact their physician and flumist.

Levy, E., S. Stan, C. Garofalo, E. E. Delvin, E. G. Seidman, and D. Menard. Immunolocalization, ontogeny, and regulation of microsomal triglyceride transfer protein in human fetal intestine. J Physiol Gastrointest Liver Physiol 280: G563G571, 2001.--To examine the multiple stages of lipoprotein packaging during development, we studied localization, ontogeny, and regulation of microsomal transfer protein MTP ; , a crucial protein for lipid transport. With the use of immunofluorescence, MTP was identified in villus and crypt epithelial cells in different regions of human fetal intestine, including colon. Staining was detected as early as the 13th wk of gestation in all gut segments and was almost entirely confined to the columnar epithelial cells of the jejunum and colon. Unlike immunofluorescence, which provides qualitative but not quantitative information on MTP signal, enzymatic assays revealed a decreasing gradient from proximal small intestine to distal, as confirmed by immunoblot. Activity of MTP in small intestinal explants cultured for different incubation periods 0, 4, 8, and 24 h ; peaked at 4 h but remained insensitive to different concentrations of oleic acid. Also, a trend toward increasing MTP activity was observed at 2022 wk of gestation. Finally, in strong contrast to jejunal efficiency, colonic explants displayed impaired lipid production, apolipoprotein biogenesis, and lipoprotein assembly, in association with poor expression of MTP. These findings provide the first evidence that human fetal gut is able to express MTP and emphasize the distinct regional distribution, regulation by oleic acid, and ontogeny of MTP. fat absorption; developmental biology; lipoprotein metabolism; intestinal transport.
