In locally advanced and metastatic breast cancer; anastrozole is available for use in the adjuvant setting in EBC, as well as in the first- and second-line treatments of locally advanced and metastatic breast cancer. An alternative endocrine treatment to tamoxifen is fulvestrant Faslodex ; , the estrogen receptor antagonist that downregulates estrogen and progesterone receptors and has no known agonist activity. Fulvestrant has been shown to be as efficacious as anastrozole in patients progressing on tamoxifen [7-9], and it has been approved in the U.S. for the treatment of postmenopausal women with hormone-receptor-positive metastatic breast cancer following progression on antiestrogen therapy. This review presents recent advances that have been made in endocrine therapy for postmenopausal women with metastatic and EBC and their subsequent impact on treatment strategies. METASTATIC BREAST CANCER ENDOCRINE THERAPY Second-Line Therapy As second-line agents in postmenopausal women with metastatic breast cancer, anastrozole and exemestane have been shown to offer significant benefits with respect to survival when compared with the progestin megestrol acetate MA ; [10-12]. Anastrozole has shown superior survival over MA with median times to death of 26.7 versus 22.5 months, respectively p 0.025 ; , and the proportions of patients surviving longer than 2 years of 56% versus 46%, respectively, at 31 months follow-up. Similarly, the overall time to death was longer for the anastrozole group than for the MA group hazard ratio [HR] 0.78, 97.5% confidence interval [CI] 0.60-1.0, p 0.025 ; [11]. Likewise, treatment with exemestane produced a greater time to progression TTP ; 4.6 months versus 3.9 months ; and was associated with a significant survival advantage in comparison with MA median time to death with exemestane had not yet been reached while with MA it was 28 months ; , although this study had a shorter overall median follow-up of 48.9 weeks [12]. In a phase III study of second-line therapy with letrozole 0.5 mg and 2.5 mg ; , the clinically approved dose 2.5 mg daily ; had a superior objective response OR ; rate 24% versus 16% ; , duration of response, time to treatment failure TTF ; 5.1 versus 3.9 months ; , and tolerability compared with MA [13]. However, in a second phase III study, the 2.5 mg dose of letrozole showed only equivalence to MA with respect to OR [14]. Both studies also reported no significant differences between the letrozole 2.5 mg ; and MA groups with respect to TTP and overall survival [13, 14]. Although the overall prevalence of mutations among clinical isolates with a levofloxacin MIC of 1 mg L remains stable, these data suggest that fluoroquinolones appear to be selecting for isolates with elevated ciprofloxacin MICs. A trend towards increased prevalence of mutations among isolates with a ciprofloxacin MIC of 2 mg L and a levofloxacin MIC of 1 mg L is probably emerging. This would suggest that there are discrepancies based on ciprofloxacin MIC, and predicting the presence of first-step mutations, and thus the possibility of treatment failure due to emergence of resistance, should be based on the combined ciprofloxacin and levofloxacin MICs as opposed to using a single marker such as levofloxacin alone. A recent review by Fuller and Low10 suggests that patient risk factors may also aid in identifying infections that have a high risk of fluoroquinolone treatment failure. Such factors include residence in a long-term care facility, hospitalization and a history of fluoroquinolone use. Recent clinical failures have highlighted the difficulty in detecting first-step mutations with the phenotypic method based on MICs using current levofloxacin breakpoints.7, 10 Reducing fluoroquinolone breakpoints, using ciprofloxacin MIC as well as levofloxacin MIC, and identifying patient risk factors may all act to reduce the risk of fluoroquinolone clinical failure due to first-step mutations in S. pneumoniae.

This study reports the effects of intrauterine administration of oestrogen and various oils on luteal lifespan in mares. Methods: A single 10mg dose of slow-release oestrogen in coconut oil was administered into the uterus of mares on day 6, 8, 10, or 14 post ovulation n 12 per group ; . Three further groups received coconut oil, peanut oil or mineral oil alone on day 10. Results & Discussion: Oestrogen in coconut oil showed peak efficacy at day 10 with luteolysis prevented in 11 12 92% ; mares; days 8 and 12 administrations were not significantly different 9 12; 75% and 10 12; 83%, respectively ; , but for days 6 and 14 administrations were lower 3 12; 25% and 6 12; 50%, respectively ; . However, oestrogen was not needed for this effect since administration of both coconut oil and peanut oil alone on day 10 gave identical results both 11 12; 92% ; , whereas only 2 12 17% ; mares treated similarly with mineral oil underwent luteostasis. Thus, the results showed that plant oils are remarkably effective in suppressing prostaglandinF2 release from the mare's endometrium.
