Randy Drake launches a new column on pharmaceuticals, "Dear Drake and Drake: Q&A Rx and OTC." Randy and Ellen Drake, authors of the annual Saunders Pharmaceutical Word Book, share their expertise on prescription and over-the-counter drugs. With his characteristic dry wit, Randy tells us right off the bat "Google doesn't spell anything, " reminding us it is search engine and may pull up thousands of misspellings. Renee Priest, in "The Hip Bone's Connected to the Thigh bone, " delights us with her clever depiction of how dictating stations for physicians are established. Richard Lederer effectively makes the point that English is the universal language. Joe Weber comes the closest he's ever been to admitting that speech recognition is NOT going to replace medical transcriptionists in our lifetime, which he has long predicted; he describes the process of "back-end recognition" as one that makes use of medical transcriptionists as medical editors and, in some cases, may increase productivity and reduce costs. Featured in this first issue of e-Perspectives is an article by John H. Dirckx, M.D., "Urines Are Cooking: Perspectives on Medical Slang and Jargon, " with a two-page spread of terms, "Translation, Please! Medical Slang and Jargon." The article grew out of our recent translation efforts while producing the new course, The SUM Program Beginning Medical Transcription, 2nd edition. Dictating physicians use so much medical slang and jargon in their dictation that medical transcriptionists are constantly called upon to know which terms to transcribe verbatim, which ones to translate into formal terms, and which ones to flag. Dr. Dirckx offers a classification of medical slang and suggests guidelines for its management. Rounding out e-Perspectives is the three-page "Update: What's New in Medicine" by Ellen Drake. These and other new terms will appear in Vera Pyle's Current Medical Terminology, 10th edition, to be published in Spring 2005. INTESTINE TRANSPLANTATION The number of intestine transplants performed in the United States is relatively small compared with other organs. In 2003, only 116 cases with data on immunosuppression were registered with the SRTR [Table 10.6a]. Because of the small total number of cases, the significance of changes in the trends of immunosuppression may be difficult to assess. Trends in Induction Therapy in Intestine Transplantation Induction therapy was utilized in 74% of intestine transplants in 2003. Two agents accounted for 82% of induction therapy. The most commonly administered induction agent was rabbit antithymocyte globulin 46% ; , followed by daclizumab 16% ; . The most notable changes in the type of induction therapy were the use of rabbit antithymocyte globulin, which increased from 31% in 2002 to 46% in 2003, and a drop in the use of basiliximab, from 13% in 2002 to less than 2% in 2003. Trends in Maintenance Immunosuppression Therapy Prior to Discharge in Intestine Transplantation Data on maintenance therapy prior to discharge were available on 103 intestine transplant recipients [Table 10.6b]. Corticosteroids were given to 67% of patients, which reflects a general downward trend in corticosteroid use in intestine transplantation. The percentage of intestine transplant recipients receiving corticosteroids decreased from 97% in 2000 to 80% in 2001 and 64% in 2002. Tacrolimus was administered to 93% of patients, while cyclosporine was used for only 1% of patients. Mycophenolate mofetil, the only antimetabolite used, was prescribed for only 9% of patients. Trends in Maintenance Immunosuppression Therapy for the First Year in Intestine Transplantation Only 80 intestine transplant recipients had evaluable immunosuppressive data between discharge and one year following transplantation. Corticosteroids were administered to 93% of patients [Table 10.6c]. Tacrolimus remained the calcineurin-inhibitor of choice, with 100% of patients receiving the drug and cyclosporine reported for only 1% of patients. Antimetabolites were rarely used 11% of patients ; . Mycophenolate mofetil was the most prescribed antimetabolite 10% ; , followed by azathioprine 1.

The risk of VT associated with hormone therapy is presented in TABLE 3. Compared with women not currently using hormones, current users of EE had no increase in VT risk OR, 0.92; 95% CI, 0.69-1.22 ; , whereas current users of CEE had an elevated risk OR, 1.65; 95% CI, 1.24-2.19 ; in analyses adjusted for matching factors and the confounding factors of race and cancer history. An algorithm, as defined by Turing and Post [23], is a finite sequence total order ; of operations directly executable by a finite state machine. For a multiprocessor architecture, composed of several finite state machines, algorithms must be specified with at least as much parallelism as the architecture. Since we want to be able to compare the implementation of an algorithm on different architectures, the algorithm graph must be specified independently of any architecture graph. Thus, we extend the notion of algorithm to an oriented hypergraph of operations graph vertices ; , which execution is partially ordered by their data dependences oriented graph edges ; . We need an hypergraph model an example is given in the top of figure 1 ; because each data dependence may have several extremities but only one origin diffusion ; . This dependence graph, also called directed acyclic graph DAG ; [10, 20], exhibits a potential parallelism: two operations which are not in data dependence relation, may be executed in any order by the same operator or simultaneously by two different operators. We deal with reactive systems which are in constant interaction with the environment that they control. This is why the algorithm operations needed to compute the output event for the actuators, from the input event from the sensors, are indefinitely repeated, once for each sampling of the sensors. In other words, the algorithm is an infinitely wide, but periodic, acyclic dependence graph, reduced by factorization to its repetition pattern [15] also called a dataflow graph ; , which is executed for each input event. In order to detect design mistakes as soon as possible in the development cycle, algorithm graphs may be produced by the compilers of high level specification languages, such as the Synchronous Languages [9] Esterel, Lustre, Signal ; , which perform formal verifications in terms of events ordering in order to prevent dead-locks. 2.3 Implementation Model and dactinomycin.

In 2005 Roche and BioCryst Pharmaceuticals, Inc. announced an exclusive license to develop and market BioCryst's phase 1 compound BCX4208 which prevents transplant rejection by a unique action on the body's own reaction to determine when to initiate immune responses and when to accept or reject newly transplanted organs. It is hoped that BCX-4208 may offer autoimmune and transplant patients a potentially more effective treatment option and daclizumab.
