FATIGUE IS ASSOCIATED WITH PERCEIVED COGNITIVE DEFICITS IN RELAPSING REMITTING MS Bonnie I. Glanz, PhD, Sandra L. Cook, RN, Karen M. Himmelberger, RN, Kristine L. Kaczor, RN, David J. Rintell, EdD, Samia J. Khoury, MD, and Howard L. Weiner, MD, Multiple Sclerosis Center, Brigham and Women's Hospital, Boston, MA Fatigue is a common and disabling symptom of MS. The association between fatigue and cognitive dysfunction in MS is unclear. This study is designed to determine the influence of fatigue on both objective and self-reported measures of cognitive functioning in patients with MS. Seventy-seven patients with clinically definite relapsing remitting MS diagnosed within the last five years participated in the study. They were administered the Modified Fatigue Impact Scale MFIS ; and the Perceived Deficits Questionnaire PDQ ; . They were also administered the Paced Auditory Serial Addition Test PASAT-3 ; , a measure of sustained attention and information processing speed. Patients included 62 females and 15 males with a mean age of 38.9 8.8 and a range of 19-60. EDSS scores had a mean of 1.6 1.5 and a range of 0-6.5. Sixty-two percent of patients were on disease modifying therapy: interferon-beta-1a 33% ; , interferon-beta-1b 9% ; , glatiramer acetate 19% ; , and cyclophosphamide 1% ; . Scores on the MFIS ranged from 0 to 76, with a mean of 27.2 + 17.7. Patients with MFIS scores above the mean comprised the High Fatigue group HF ; and patients with MFIS scores below the mean comprised the Low Fatigue group LF ; . The HF group reported significantly more cognitive complaints than the LF group P .001 ; . On the PASAT-3, HF and LF mean scores did not differ significantly from established norms. There was no statistical difference in PASAT-3 performance between the HF and LF groups. Patients with early relapsing remitting MS reported varying levels of fatigue. Patients with higher levels of fatigue reported more cognitive difficulties. Despite their subjective complaints, patients with higher levels of fatigue did not perform differently than patients with lower levels of fatigue on a cognitive task. These findings suggest that fatigue may contribute to the inability of MS patients to accurately appraise their own cognitive functioning.
N.B. Cyclizine should be used with caution in patients with severe heart failure. In such patients, cyclizine may cause a fall in cardiac output associated with an increase in heart rate, mean arterial pressure and pulmonary wedge pressure 5.

A combination of Caroli's disease bouts of cholangitis, hepatolithiasis, and gallbladder stones ; and those of congenital hepatic fibrosis portal hypertension ; . Clinical progression and presentation of Caroli's syndrome is highly variable and symptoms may appear early or late during life. The main consequences of congenital hepatic fibrosis are portal hypertension and the development of oesophageal varices. Portal hypertension may result in hematemesis or melena. In the majority of patients, portal hypertension will not be present or will appear only later in the disease evolution[10]. The late appearance of these symptoms in patients with Caroli's disease sug gests that congenital hepatic fibrosis in Caroli's disease is dynamic and progressive[11]. Bile stagnation and hepatolithiasis explain the recurrent cholangitis which dominates the clinical course and which is the principal cause of morbidity and mortality. Chronic abdominal pain, pancreatitis resulting from biliary stones and liver abscess are other disease complications[10] and cycloserine.

In spite of the 10 38-fold difference between SCLM and FDA-PI, this latter was acceptable for our purposes since: a ; the use of FDA-PI for 5-nitroimidazole comparisons in structure-activity assays see next section ; gave qualitatively similar results to SCLM data not shown ; , i.e. determinations are distinct only in a sense of magnitude and b ; FDA-PI has several advantages in terms of cost, performance time and possible automization. FDA-PI, which is a direct, easier and faster cell-by-cell assay was used for the remainder of this study instead of SCLM to evaluate the effect of 5-nitroimidazoles on Giardia viability. KASTOR SMASHES ARS; AMERICAN DISTANCE RUNNERS SEND A MESSAGE Led by Deena Kastor's four American records in the span of three weeks including a victory and AR at the Flora London Marathon American distance runners proved this spring that they are among the world's best. At the Flora London Marathon on April 23, Kastor had predicted a win and an American record, and she more than succeeded on both counts. Response to rhIL-11 were continuous during the 7-day treatment period and gradually abated, but were still detectable several days after treatment ended. Platelet counts increased by 78% 264 103 to 468 70 106 mL by day 10 ; , and the total fibrinogen increased by 150% 154 21 to 385 40 mg dL by day 7 ; during rhIL-11 treatment complete data not shown ; . Yet another difference between the effects of the 2 treatments becomes apparent when the VWF multimeric composition is analyzed as illustrated in Figure 2. While no obvious change in multimer distribution is observed when the dogs are treated with rhIL-11, DDAVP treatment results in a very clear shift toward release of very-high-molecular-weight VWF multimers. Neither platelet count nor fibrinogen level was significantly changed when the dogs were treated with DDAVP data not shown and dacarbazine.

The pilot phase of this trial was started in September 2001 with 23 centers in Germany, and enrolled 286 patients within 1 year.
