Miller, MD: This question is an excellent example of where there are multiple "right" answers depending on the individual's situation. All areas of disease are the same cancer and should respond the same way to treatment. In most cases, it is unnecessary to repeat tests that evaluate all sites of disease at each visit. I generally follow the one or two areas of disease that are easiest. If there is a palpable nodule or lymph node, a simple physical examination and careful history might be all that is needed. If there is nothing on physical exam, I pick the easiest and less expensive, least cumbersome ; X-ray that clearly shows disease. The products of these enzymes, eicosanoids, are important factors to regulate blood pressure and renal sodium transport. However, at what level the down-regulation of these enzymes occurs and whether the HF diet has any effects in female rats are not known. This study was designed to examine blood pressure, renal CYP enzyme activities, and sodium balance in male and female rats fed the HF diet or regular rat chow. HF male rats showed a higher blood pressure after 6 10 week treatment and a decrease of renal cortical -hydroxylase activity by 46% and epoxygenase activity by 46% after 10-week treatment, but no blood pressure and activity change were observed in female rats. HF diet treatment also caused a 30% decrease in proximal tubular -hydroxylase activity 102 6 vs 71 pmol min mg, n 4 ; and a 38% decrease in epoxygenase activity 42 2 vs pmol min mg, n 4 ; in male rats. Moreover, HF diet treatment in male rats caused an increase in cumulative sodium balance from -1.7 0.5 to 1.6 0.3 mmol day, n 6 ; and an elevation of blood pressure from 121 2 to 136 4 mmHg, n 6 ; , whereas no change in cumulative sodium balance was observed in female rats. In conclusion, there is gender-different regulation of renal eicosanoid synthesis and blood pressure in HF rats. We speculate that the haemorrhages which occurred in our patients were more likely to have occurred during lesioning rather than during recording, since the associated deterioration in clinical condition occurred postoperatively in the intensive care unit rather that intraoperatively at the time of electrophysiological recording. Moreover, experience with deep brain stimulation, including initial microelectrode recording in many cases, and also cases in which no deliberate lesion is performed, has revealed a very low rate of intracerebral complications. Minor and transient complications which did not interfere with the patients' quality of life were common 39% ; but these patients considered that the improvement in motor function, particularly dyskinesias, outweighed the disability associated with their complications. In our series, as in others Baron et al., 1996; Johansson et al., 1997; Lang et al., 1997 ; , transient worsening of speech was noted, and no patients noted improvement in speech. Also, one of four patients who received adrenal medullary grafts at Columbia University Fazzini et al., 1991 ; subsequently died 1 year postoperatively from a glioblastoma multiforme two separate foci in right frontal lobe and one in left thalamus; the graft was in left caudate ; . We have never encountered another patient with Parkinson's disease who has developed a malignant glioma, but are unable to determine whether these three instances are coincidental or are in some way related to basal ganglia surgery. Five of our patients complained of worsened gait postoperatively, although this was not evident on examination. We encountered similar problems in the interpretation of the outcome to pallidotomy in several patients who reported that motor performance had deteriorated on the non-operated side. While it is possible that ipsilateral symptomatic deteriorations were a true consequence of medial pallidotomy, these phenomena are perhaps more likely explained by disease and cyclizine.
