Consistent with the MIC data, GPM259 and GPM265 are more resistant to DCS than the wild-type mc2155 but less resistant than the Alr-overproducing strain GPM14 Fig. 5 ; . Furthermore, this type of intermediate level of resistance to DCS is not related to Alr, since crude extracts from both GPM259 and GPM265 contained a wild-type level of Alr activity Fig. 3B ; . Therefore, overproduction of the Ddl enzymes leads to a DCS resistance phenotype in M. smegmatis. Consecutive non-duplicate aerobic gram-negative bacteria isolated from patients with nosocomial infections in 10 ICUs in different parts of Russia were studied. It contacts the ribosomes or are resistant at the ribosomes themselves 10 ; . Therefore, killing does not occur, and no synergism is apparent even though there is increased penetration of the aminoglycoside 20 ; . About 40% of strains of enterococci have been shown to be highly resistant to streptomycin, whereas none have been shown to be highly resistant to gentamicin 19 ; . Thus, gentamicin in combination with penicillin or ampicillin shows a synergistic effect against all strains in vitro 14, 15 ; . The superior killing of the moderately streptomycin-resistant enterococci MIC, -2, 000 Ag ml ; by ampicillin plus gentamicin, however, was unexpected because these strains were killed synergistically by either streptomycin or gentamicin in combination with the penicillin. MICs indicated that the enterococci were more susceptible to gentamicin than to streptomycin. This may reflect a greater sensitivity of the ribosomes to the antibiotic rather than, or in addition to, an increased ability of this aminoglycoside to penetrate the cell wall. This increase in the activity of gentamicin apparently overcompensates for the low serum levels of the antibiotic. In addition to congenital and rheumatic valvular disease, degenerative heart disease has been described as a relatively frequent underlying lesion in bacterial endocarditis 11, 18 ; . Patients with mitral valve prolapse Barlow's syndrome ; also have an increased incidence of endocarditis 5 ; . This cardiac anomaly is very common, occurring in 0.5 to 21% of the adult population 12, 17; 0. R. Brown, R. E. Kloster, and H. DeMots, Circulation 52[Suppl. II]: 77, 1975 ; . Therefore, the number of patients at risk for developing endocarditis and who may benefit from antibiotic prophylaxis after invasive procedures of the genitourinary or gastrointestinal tract is relatively large. The current recommendations require the initial antibiotics to be repeated for two or three additional doses over a 24-h period, depending upon whether streptomycin or amikacin is the aminoglycoside used in the regimen, respectively. This is often impractical and may require admission to the hospital. Prophylaxis which can be administered as a single dose to many of these ambulatory patients is imperative. Although this study does not indicate that gentamicin in combination with ampicillin is effective when administered in this manner, it does suggest that it is the most promising of the currently recommended regimens to meet this requirement.
